Primary Mental Health Nexus Evaluation

At Essential Veteran Services, we provide primary mental health nexus evaluations that are structured to meet VA evidentiary standards. These evaluations assess whether a mental health condition is at least as likely as not related to military service and are often foundational for Disability Benefits Questionnaires and nexus opinions.


EVS provides primary mental health nexus evaluations for conditions including:


  • Post-Traumatic Stress Disorder (PTSD)
  • Anxiety Disorders
  • Depressive Disorders
  • And related mental health conditions


Each evaluation includes a comprehensive clinical record review and clear documentation written in VA-appropriate language, integrating clinical expertise with lived understanding of military culture.

Secondary/Aggravated Conditions Nexus Evaluations

Secondary and/or aggravated conditions evaluations examine whether a service-connected condition has caused or worsened another condition over time. These evaluations are commonly used when a Veteran’s claim has been denied due to lack of a clearly documented connection between conditions.


Essential Veteran Services provides secondary and aggravation nexus evaluations for the following conditions:


  • Sleep Apnea
  • Gastroesophageal Reflux Disease (GERD)
  • Migraine Headaches, and related conditions


These evaluations focus on clinical record review, symptom chronology, and established psychological and psychophysiological considerations, and are written to address VA standards for secondary service connection. Availability is limited to the conditions listed above, and not all cases will be appropriate for this type of evaluation.

Dr. Sheena Glover is an Air Force veteran and licensed psychologist who knows the culture of service from the inside. Like many who served, she understands how asking for help can feel like weakness or letting down the team. That perspective shapes how she shows up for every veteran she works with, never as a distant evaluator, but as someone who has walked a similar path.


For more than a decade, Dr. Glover has dedicated her career to trauma-focused care, specializing in military sexual trauma (MST) and post-traumatic stress disorder (PTSD). With over five years of experience conducting VA mental health evaluations, she brings both clinical expertise and personal understanding to every claim.


As the founder of Essential Veteran Services, Dr. Glover’s priority is advocacy. Yes. All independent medical opinion (IMO) evaluations are conducted virtually.


What to Expect

Prior to your scheduled appointment, you will receive a secure video link with instructions for joining your session. You can complete your evaluation from any private location with a stable internet connection and a device capable of video conferencing.


What Is Covered During the Session

Your virtual evaluation may include:

  • A comprehensive clinical interview
  • Review and discussion of relevant history and symptoms
  • Discussion of Nexus Letters related to your service-connected claims


Completion or review of the Disability Benefits Questionnaire (DBQ), when applicable


Preparing for your mental health evaluation helps ensure the appointment runs smoothly and allows the clinician to focus on understanding your experiences and symptoms.


Complete Your Intake Form in Advance

Before your appointment, you will receive a secure intake form. This form gathers essential background information about your history, symptoms, and military service. Completing it fully and accurately prior to your session is required.


Upload Supporting Documents (If Applicable)

If you are working with a coach, attorney, or representative, coordinate with them to ensure that any supporting materials are uploaded to your secure client portal before your appointment. These may include:

  1. Disability Benefits Questionnaires (DBQs)
  2. Nexus Letters
  3. Prior mental health records or evaluations
  4. Relevant service or medical documentation
  5. Uploading these materials in advance is especially important when an independent medical opinion (IMO) is being requested to support a service-connection claim.
  6. Bring a Valid Photo ID


On the day of your appointment, the only physical item you need to bring is a valid government-issued photo ID. This is required to verify identity and access veteran services.


After your independent medical opinion (IMO) evaluation is completed, your Nexus Letter and Disability Benefits Questionnaire (DBQ), when applicable, are typically provided within 48 hours.


What This Timeline Includes

This timeframe allows for:

  • Review of your clinical interview and records
  • Completion of VA-compliant documentation
  • Quality review to ensure accuracy and clarity


Our goal is to deliver thorough, well-supported documentation as efficiently as possible to support your service-connection claim.


While both standard nexus letters and secondary service connection evaluations provide medical opinions for VA claims, they serve different purposes and address different types of relationships.


Standard (Primary) Nexus Letter

A standard nexus letter focuses on whether a single condition is directly related to military service. It evaluates:

  • The claimed condition
  • In-service events, exposures, or stressors
  • Whether the condition is at least as likely as not related to service

This type of letter supports direct (primary) service connection.


Secondary Service Connection Evaluation

A secondary service connection evaluation focuses on the relationship between two conditions. It evaluates:

  • An already service-connected condition
  • A second condition being claimed
  • Whether the service-connected condition caused or permanently worsened the second condition

This type of evaluation supports secondary or aggravation service connection.


A secondary service connection opinion evaluates whether a service-connected condition has caused or aggravated another medical or mental health condition.


This type of opinion is used when a Veteran already has at least one condition recognized as service-connected and later develops an additional condition that is medically related to the original disability.


What the Opinion Addresses

A secondary service connection opinion examines:

  • The existing service-connected condition
  • The additional condition being claimed

Whether medical evidence supports a causal or aggravating relationship between the two

The clinician reviews medical records, relevant history, and current symptoms to determine whether it is at least as likely as not that the service-connected condition caused or permanently worsened the secondary condition.


Why Secondary Opinions Matter

Secondary service connection opinions can significantly strengthen a claim by clearly documenting how conditions are interconnected. This is especially important when symptoms develop over time or as a consequence of long-standing medical or mental health issues.


Some Veterans develop additional medical or mental health conditions after, or alongside, an already service-connected disability. In these situations, a secondary service connection evaluation can provide the medical clarification needed to support your claim.


When This Evaluation May Be Appropriate

A secondary evaluation may be helpful if:

  • You have an established service-connected condition
  • You later developed another condition that may be medically related
  • Your prior claim was denied due to a lack of a clearly documented connection
  • The VA requested clarification regarding causation or aggravation


What the Evaluation Does

A secondary evaluation carefully examines whether one condition has caused or permanently worsened another condition. The clinician conducts a thorough record review and applies medical reasoning to determine whether the relationship meets the VA standard of “at least as likely as not.”

By clearly documenting how conditions are interconnected, this type of evaluation strengthens the evidentiary foundation of your claim.


Our evaluation fees reflect the time, clinical expertise, and documentation required to produce thorough, VA-compliant medical opinions and supporting documentation.


Mental Health Nexus Evaluation

The fee for a Mental Health Nexus Evaluation is $900. 


This evaluation includes a comprehensive clinical interview and record review, completion of the required Disability Benefits Questionnaire (DBQ), and a detailed Nexus Letter establishing service connection. Payment is required in advance to secure the independent medical opinion.


Secondary or Aggravation Nexus Evaluations

Secondary or aggravation nexus evaluations are offered for several commonly claimed conditions. The current evaluation fees are as follows: 


Sleep Apnea: $900. 

GERD: $750. 

Erectile Dysfunction: $750. 

Migraines: $650. 


These evaluations include clinical review and medical record analysis, a Nexus Letter addressing secondary or aggravation service connection, and the required supporting documentation for VA consideration. Payment is required in advance.


Rebuttal Letters

For all evaluations, rebuttal letters are provided at no additional cost if the VA requests clarification or issues a denial based on inadequate reasoning. This helps ensure continuity of support throughout the claims process.


Dr. Glover participates in  PSYPACT (Psychology Interjurisdictional Compact), which allows licensed psychologists to provide telepsychology services across participating U.S. states.


What This Means for You

Because of PSYPACT participation, Dr. Glover can conduct virtual mental health evaluations for Veterans located in 43 participating states. This allows eligible Veterans to receive:

  • Independent medical opinion (IMO) evaluations
  • Completion of the Disability Benefits Questionnaire (DBQ), when applicable
  • Nexus Letters supporting service-connected claims
